New clients can download the initial paperwork for the first session, (please print these out, complete them, and bring them with you to the first session).
New clients who wish to use their health insurance must contact their insurance company and confirm they have current mental health benefits for outpatient office visits and ascertain their co-pay amount and whether they have a current deductible before the initial appointment. Please note this information in the appropriate places in the initial paperwork.
Please bring your insurance card and credit card with you to the first session.
